Fees and Payments We require that you pay for services
at the time of each visit. An estimate of the charge for any
procedure or lab work will be given when requested, and we encourage
you to ask. If you have a large bill or current financial
difficulties, you should speak with the administrative assistant or
head receptionist to work out a suitable arrangement. If your
account becomes past due and there is no valid reason for your
payment delay, appropriate action will be taken to recover the
amount due. If your account is ever turned over to collections, you
will then be responsible also for the
collection fees.
The charge for a non-sufficient check
is $25.00. We accept payments on your account with approved checks,
cash, Visa, MasterCard, American Express,
and Discover.
Elective Surgery Financial arrangements for elective
surgery must be made prior to the surgery through our account
manager.
Insurance Health insurance policies are contracts between you and your
insurance company. As a courtesy to our patients, we are a specialty
provider for most PPO plans, and we will file for you. You will be
responsible for any referrals needed, your co-pay, and meeting your
calendar deductible. Any questions regarding your coverage should be
directed to your insurance company rather than
this office.
Confidentiality In order to ensure confidentiality, we
do not give out medical information about you to others. Even close
relatives or insurance companies cannot obtain this information
without your consent.
